    ​A school nurse:

    • Provides urgent care with a focus on prevention and wellness.
    • Serves as a medical liaison between the school, students, families and health care providers.
    • Oversees state-mandated on-site vision and hearing screening.
    • Monitors student immunization status.

    Please contact me if your child has a health condition that might impact their learning.

    Does your child need to take medication at school (every day, short term prescription, or as needed)? Complete the Authorization for Medications to be Given at School form (parent completes/signs top half; healthcare provider completes/signs/dates lower half; fax or bring to school).

    Helpful Immunization Record-Keeping Tips for Your Family

    Did you know that you can manage your family’s immunization records securely online and save yourself a call/visit to your healthcare provider or school? Register for this service through the Washington State Public Health Department and access the records any time you need them. You can also print your child’s Certificate of Immunization Status for school and child care entry. Check it out here
